Age Requirements

Kindergarten: Child must be 5 years of age on or before 9/1

First Grade: Child must be 6 years of age on or before 9/1

 

Proof of Age

Certified Birth Certificate

Medical Record

School Physical Examination

First time enrolled in a Florida School. A physical examination within the last 12 months,

prior to enrollment in school would also be considered valid proof.

Other Information that may be required

Student?s Social Security Number

A copy of the last report card or a withdrawal form

Proof of residence
